We cordially invite you for BMM Ganeshotsav 2021 program.

Please note that

• Only 250 people are allowed to attend
• Each person 5 years and older requires a ticket to attend
• Each non refundable ticket is only $2.00 per person to confirm your RSVP
• Ticket purchase confirms you agree to our waiver document.

We request you to read below waiver document and then purchase tickets below to reserve your spot for the program.

I understand that attending the Baltimore Marathi Mandal Ganeshostav 2021 event on Sept 19 2021 during this time of COVID 19 may pose certain risk to myself and my family. I and my family members accompanying me are willingly participating in the event. I agree for following 5 items by paying the event ticket

1) I am of sound mind and good health and will comply with all the instructions given to me at the venue. 

2) I will ensure that I and my family members accompanying me at the event have been fully vaccinated for COVID-19 –OR– will have a negative COVID-19 test within the 3 days prior to attending the event

3) I confirm that I will not come to the event if I believe I have been exposed to someone with a suspected and/or confirmed case of COVID-19 in the 14 days prior to the event.

4) I confirm that I will not come to the event if I have been diagnosed with Coronavirus/COVID-19 by state or local public health authorities in the last 14 days prior to the event.

5) I being of legal age hereby release and exonerate Baltimore Marathi Mandal of any loss of personal belongings, any personal injury, and any claim resulting from attending the event.
